Consent Form and Privacy Policy

I hereby voluntarily consent to my employer (the data controller) processing and storing personal data about me. In practice, it will be the consulting firm Health Group (the data processor) that will collect, process, and store personal data about you on behalf of your employer.

In connection with employee satisfaction surveys, workplace assessments, and other analytical work, the personal data collected will include:

General information, as specified in Article 6 of the General Data Protection Regulation: Name, email address, and department (if applicable).
Special categories of personal data, as defined in Article 9 of the General Data Protection Regulation: Responses from survey data regarding the physical and psychological work environment, health, sick leave, and medical information.

The purpose of transferring your personal data to Health Group is to enable us to prepare legally required occupational health and safety reports, as well as other reports for your employer, in an anonymized form.

You have the right to withdraw your consent at any time. You can do so by contacting Health Group by phone at 70201626 or by email at info@healthgroup.dk. If you choose to withdraw your consent, it will not affect the lawfulness of the processing of your personal data based on your previously given consent, up until the time of withdrawal. If you withdraw your consent, it will therefore only take effect from that point forward.

Your rights

In addition to your right to access the information that Health Group processes about you, you also have the following rights under the General Data Protection Regulation:

the right to have inaccurate information about yourself corrected
the right to have information about you deleted before the scheduled deletion date
the right to restrict the processing of your personal data
the right to object to the otherwise lawful processing of your personal data
the right to receive your personal data in a structured, commonly used, and machine-readable format, and to have that personal data transmitted from one data controller to another without hindrance

Your data will be automatically deleted 6 months after the survey is completed, unless we have agreed otherwise with your employer. Yourdata will not be disclosed to third parties, but will be included at an aggregate and anonymized level in the report prepared for your employer. No automated profiling is performed.
